Aviation officials have blamed the pilot for a 2004 accident that killed two men when a small plane crashed into an unlit television tower near Lake Wales.
Just hours before the Sept. 11, 2004, crash, a nearby car accident knocked out electricity in the area, including power to the 520-foot tower, the report said.
David Wilkes, 55, of Cleveland, Mississippi, and Harold Miller, 53, of Sandwich, Illinois, were spraying insecticide from the plane when it crashed into the tower at a citrus grove near State Road 60 about 5:30 a.m.
According to a report released Thursday by the National Transportation Safety Board Wilkes, pilot of the Piper Aztec twin-engine plane, did not obtain a weather or preflight briefing before the flight. A briefing also would have told him about the power outage.
